She is probably talking to the guy ahead of her, and she is either cold and holding her scarf up to her ear, or using her scarf as a shield from the wind, or she could be aware that someone is there with a camera and she could just be camera shy and doesn’t want to be on film and using the scarf to cover her face

It’s a hearing aid, she can be moving her mouth because shes chewing something or repeating something in her head to not forget, the laugh could be her listening to the director with the hearing aid say something to another person off set or to her.
